Requirement to participate in weekend On-call Roster
Please note that this position will be filled at either RN2C or AHP2 classification.
RN2C: Provide nursing and/or midwifery services in a variety of health service settings which has been consolidated by experience and/or further study with staff at this level developing from competent to proficient practitioners.
Accepts accountability for their own practice standards, activities delegated to others and the guidance and development of less experienced staff.
Provide support to the Nursing/Midwifery Nurse/Midwife Unit Manager or equivalent in the leadership of nurses/midwives in the ward/unit/service.
Employees in this role will undertake a portfolio within which they will:
Promote continuity and consistency of care in collaboration with other AN/MUM and the Nurse/Midwife Unit Manager or equivalent;
Assist the Nurse/Midwife Unit Manager or equivalent in the implementation of practice changes; and
Assist the Nurse/Midwife Unit Manager or equivalent in undertaking ward/unit/service management responsibilities, eg. Performance management processes, recruitment, staffing, leave management, rostering, work allocation and attendance management; financial and supplies planning and monitoring.
AHP2: Clinical Support Officer is accountable to the Program Manager, Country Referral Unit (CRU), Country Health SA for active participation in the Country Referral Unit Project, throughout Regional South Australia, on behalf of the 6 Local Health Networks – Barossa Hills Fleurieu (BHFLHN), Eyre and Far North (EFNLHN), Flinders and Upper North (FUNLHN), Limestone Coast (LCLHN), Riverland Mallee Coorong (RMCLHN), Yorke and Northern (YNLHN).
The main components to the role are:
Providing clinical knowledge and expertise to assist Intake Officers and MAC staff in their intake role
Effectively manage the referrals, related documentation and data of the CHSA Transition Care (TCP) and Country Home Link (CHL), Community and Home Services Program (CHSP), My Aged Care Portal and Aged Care Assessment Program (ACAP) in line with SA Health and DoHA guidelines
Supporting consistent provision of quality TCP, CHL, CHSP, MAC and ACAT and allied health referral and allocation processes within the Country Referral Unit Project
Enhancement of skills and knowledge of Intake Officers and MAC staff, in providing needs assessments.
Strong understanding of the Community and Home Services Program (CHSP) Sector, My Aged Care Portal, Aged Care Assessment Program, and Community Services Sector.
